The Medicare website has a list of items that may help you determine what facility is best for you. Visit medicare.gov to find the full checklist or review this partial list below.

Consider the following when you visit a care facility:

Basic Information

  • Does the skilled nursing facility (SNF) provides the skilled care you need, and is a bed available?
  • Does the SNF have the special services needed in a separate unit (e.g. dementia, ventilator, or rehabilitation), and is a bed available?
  • Is the SNF located close enough for friends and family to visit?

Resident Appearance

  • Are residents clean, appropriately dressed for the season or time of day, and well groomed?

Living Spaces

  • Does the SNF appear clean and well kept?
  • Is the temperature in the SNF comfortable for residents?

Staff

  • Does the relationship between the staff and the residents appear to be warm, polite, and respectful?
  • Do all staff wear name tags?
  • Is there a full-time Registered Nurse (RN) in the SNF at all times, other than the Director of Nursing?
Residents’ Rooms

  • Are residents allowed to have personal belongings and/or furniture in their rooms?
  • Does each resident have personal storage space (closet and drawers) in his or her room?
  • Do residents have access to a personal telephone and television?
  • Do residents have a choice of roommates?

Activities

  • Are residents, including those who are unable to leave their rooms, able to choose to take part in a variety of activities?
  • Does the SNF have outdoor areas for resident use and staff to help residents go outside?

Safety & Care

  • Do residents get preventive care, like a yearly flu shot, to help keep them healthy?
  • Can residents still see their regular doctors?
  • Are care plan meetings offered for residents and family members at convenient times, whenever possible?
  • Has the SNF corrected all deficiencies (failure to meet one or more Federal or State requirements) on its last state inspection report?
